You have to unlock Command missions first, then run 5 different command missions, then you have your squadron run a flagged routine mission. Done. The flagged mission appears in the bottom of the middle tab. Its a level 50 mission.

How do I join a grand company?
Grand Companies are organizations that players will join at around level 20 by completing the main story quests. There are three grand companies, The Maelstrom from Limsa Lominsa, The Immortal Flames from Ul’dah, and The Order of the Twin Adder from Gridania.
How do I join a free company?
The player can apply to found a Free Company after they have reached leveled a Grand Company. In order to start a Free Company, players must talk to the Free Company administrator and select the “Found a Free Company” option. The player then gets to choose the preferred name and tag for the FC.
